Am wanting to send an item that is worth £5 abroad. The Royal Mail website is quoting £22.50 which includes cover of up to £20.
Does this "cover" in the event the item goes missing include the cost of postage (ie. if the worst happens would I get back £5 plus the £22.50 paid, or would I just get a £20 total)?

The cover is additional to the cost of postage. See:
https://www.royalmail.com/international-compensation" onclick="window.open(this.href);return false;
Note that you will need proof of postage from the Post Office to make any claim, and proof of the items value to claim for that element.
