Might not be right, but its one possible avenue to an explanation, I'd expect it to be something along this line, too much automation that has been defeated.
Within weeks there were reports that the Chinese could do this.I think somewhere in the 2d barcode there is a unique identifier and they managed to create the IDs themselves.
