fact finding
When somebody has had a fact finding and the case has then gone to conduct code should there be notes from the fact finding?

By my understanding you should have had a copy of the notes taken from the fact finding for you to accept and/or modify as necessary before things went any further.
Good security means trying to limit the damage a Trusted role can do
