Help! five years history work

Guys! I am new here

I am trying to apply a position of Christmas casual in greenford centre.

here they asked me for the 5 years history work...

I have done them

but i dont know where i am going wrong



19 September 2012 - Present - filing clerk

28 June 2012 -18 September 2012 - - Summer holidays

25 September 2011 -27 June 2012 - bsc computing - 3rd year

24 September 2011- present - voluntary charity assistant

16 June 2011 - 23 September 2011 - Summer holiday

30 September 2010 - 15 June 2011 - BSC computing - 2nd year

15 July 2010 - 29 September 2010 - Summer Holiday

28 September 2009 - 14 July 2010 - Attending Colleges

16 July 2009 - 27 September 2009 - Summer Holidays

28 September 2008 - 15 July 2009 - Attending College

and still they are asking for "Records are required for at least 5 year(s) with a maximum of 14 day(s) between entries"

Help please

The form may have changed but I reckon it only accepts one "to present" entry (the top one) I think I put my charity work in with the other postions eg
mar 2nd 2010 - present ....................... clerical assistant plus charity work
jan 2nd 2010 - mar 1st 2010................. jobseeking plus charity work

so I'd try adding your volunteering to your Bsc 3, filing clerk, and recent Summer Holiday entries, and drop the separate entry for the voluntary work.

The fact that you've only included four years history might be the crux of the matter...

Think you need to go back to October 2007.
