Ermm...What is it - i've 4.5 days - do i just choose the bold and underlined dates or something?

Usually the bold weeks/days mean those dates have been taken.
And the .5 usually gets carried over to the next year.

From memory, on the Annual Leave Card aren't the bold/underlined weeks Christmas pressure?

Hmm in our office we can't book less than a full week until all the full weeks have been decided, we also have to carry less than 3 days over to keep them for "emergency days"
