I’m on the 12 week rate.
As I’ve said but I haven’t done a shift in 8 weeks, I’ve read on here that I lose the rate after 6 weeks, is this correct?

unfortunately yes. To avoid losing the higher rate you can apply for holiday pay, minimum 4 hours & max 40 hours, providing you have the hours to claim!
