Hand canceled letters?

Can someone tell me in what circumstances a standard letter with a first class stamp would have it's stamp canceled by hand? Office postcode and date.
Once in the system would it not go through any machinery and who would do the canceling?

When I visit the post office with a letter she sticks the stamp on & then stamps it?
