Halfway through a 2 year serious offence penalty, if I were to get another penalty , totally unrelated to first offence ie it was H@S issue , could they dismisss

Maybe the offence might not be classed as a serious offence without more information hard to tell,
But if its a serious offence it will be down to the manager to decide and decide if he's going to use it with the 2 year serious offence.
Fingers crossed its not serious bud

First offence was breach of H@S , no seatbelt , 2nd one was late delivery of special by 2mins !!!

Are you suggesting you could be on a 2 year serious for one offence, another two year serious for another and possibly a third for another?
3 two year seriouses? Would they run concurrently?
I don't think that is how the system works.

And yes they could in theory sack you for repeated infractions. But they probably won't.

It is not necessary for there to be a single act amounting to gross misconduct before an employer can dismiss fairly without notice. Conduct amounting to gross misconduct is conduct which undermines trust and confidence between employer and employee. That conduct can comprise a single act or several acts over a period of time. It is quite possible for a series of acts demonstrating a pattern of behaviour to be of sufficient seriousness that it undermines trust and confidence.
