I received my "persons specifically at risk form" the union rep couldn't tell me how to interpret the report, can anyone give me any help, I would appreciate it

As you can see I've asked about this before, I've asked my area rep and he says he will get back to me, so far I've had the report done and I still don't know what good it does me. If anyone can explain it to me I will be very grateful.
