I would hope your doctor would be able to put the date on a sick note to include and dates you need. May depend on your doctor.

Been signed off work with a chest infection for a week since last Tuesday. Now I have another problem, since Thursday my foot has become red and very swollen. Went to GP on Friday and she said it might be an infection and took a blood sample. Due to go back to work tomorrow, and cannot get another sick note until Thursday. Will my absence tomorrow be treated as unauthorised?

No problem your GP will back date your sick note make sure to tell her .As for work give your line manager a ring and tell him about your foot and tell him you will see your GP as soon as possible for a sick note .RM know you can't just see a Doctor and have to make an appointment.

Just got a call from him saying he has looked on his laptop at my sick record and wants me to go in to work tomorrow to discuss it with him. Can he do this?

He can ask.
You can politely decline citing your horribly swollen and extremely painful foot.
