I have been told I can get a copy of my case notes from ATOS Sheffield. Does anyone know their address and phone number please?

If this was a recent assesment done on yourself or over the phone, your manager should give you a hard copy as atos normally send the notes by email to the manager that requested the assesment.he should give it to you if you request it
