Incorrect sicknote date

Had an accident at home on Monday, 25th. GP gave me a sicknote for 2 weeks yesterday, BUT...I only noticed last night that he had dated it from the 18th.
(I did initially ask for a Self Cert for the first week, but he said it would be easier for him do it this way. My normal GP is on A/L, so he's a stand-in.)

I want to hand this into my office on Monday, but what's the best for me thing to do:
1. Take sick note to work as it is on Monday, pointing out the human error?
2. Ring surgery to tell them on Monday, to see if it's still valid?
3. Return to the surgery on Monday, to get note altered or replaced?

I need to get this into work on Monday, as per RM regulations, so need some urgent suggestions asap.

Have you got a writing room who deal with the duties? If so I would phone them on monday to let them know the situation. Then go back to your GP and get them to alter it or issue a new one. As long as you let everyone know what is happening it shouldn't be a problem.
