In this day and age I would be careful saying that the DOM is hassling you, as they sometimes say that they are just enquiring after your health.
If you genuinely believe you are being harassed then the only thing you can do is note the date and time of everything that happens with regard to this sickness, if possible also tape record any meetings or phone calls.
A plain notebook would suffice and start with details of what has happened already from memory. For example
14 Jan 08 - 1130 - Sprained Ankle whilst on delivery
14 Jan 08 - Approx 1145 Called DOM and advised him of situation and that I could not complete.
14 Jan 08 - Approx 1200 relief turned up and I briefed him on what needed completing.
14 Jan 08 - Approx 1300 (1pm) Attended casualty.
And keep filling in all the pertinent points till you get to today.
Any entries from that point onward especially meeting with managers it might be worth getting them to sign it in case there is a dispute further down the line. Or take in a rep and get him to initial the entry.
Golden rule is start logging incidents from this point onward.
