Im in my 2nd year and i totally forgot to fill out and send in my leave form to request the 3 weeks they force you to book in advance. Ive just noticed on the people app they have booked those 3 weeks themselves and im not too satisfied with the dates they have chosen.
Can i change this? If so, how do i go about rectifying this?

You can ask the manager, but leave slots fill up fast and you would need to pick a week that has availability. If in the CWU your local rep may help or advise you.

best bet is if you have any particular dates in mind then ask a colleague if they don't mind swapping, i've already got 3 weeks of my leave booked but nothing planned in regards to going away as yet, school holidays will be hard though,
also keep an eye on if/when anyone leaves and check what their dates were, if fast you could always nip in a grab an open slot

All the employer has to do is offer the maximum 5.6 week's annual leave entitlement within the current leave year
They will try to accommodate specific leave request dates but because of business needs, can dictate when you take that annual leave entitlement
And as you have already stated, you forgot to send those requests to management prior to allocation
