Google for a template letter and edit it with your details. Send it off to HMRC. Simples.
All you are saying is, I work for RM & need to wash my work uniform. I want to claim back xx years tax allowance for washing my uniform.

It was automatically applied when I joined. Check your tax code, you are probably already getting it.
