Ref Managers issuing stage warnings without notes

I had a stage warning 3 weeks ago, I never took a rep in with me because he said it was going to be non issue. Now last week I pulled up my manager and asked for stage review notes to be signed, his response was I've got 5 in front of you to type up. Now in my experience I've had a stage warning in the past on PsP yet I had the notes showing that its not issued. What's my position later down the line that I need to go sick and it triggers the next stage, I have no written evidence, though what do they do with their copy of the supposedly signed notes?

Well see your rep but you could put a grievance in about the notes or lack of them if the manager is not doing his job he could be sacked could he not I am sure HR would love to know if they are under performing in their duties as a manager it could save HR a lot of money!

Did you sign anything? If not, no evidence of meeting ever taking place. If you did what were those papers?
