Stage 3?

Can a stage 3 be issued, if you only on a stage one, and never had meeting or mention of a stage 2 ?

Stage 3 is a meeting regarding dismissal. Its not issued....you get sacked

No. They may try to, but that's why Area Reps deal with Stage 3s. Even if the worst happens an ET would likely support your claim as if would be a failure of RNMs own processes and would, if you can prove it, be automatically an unfair dismissal or unfair application of the internal process.

Someone working with me has got away with 2 stage threes this year

I got away with 2 stage 3s thanks to mental health problems (well not thanks to but they're terrified of getting done for sacking someone with them)
