SD Scanning failures - DISMISSAL ?

Colleague received a 2 year serious warning last summer for Special Delivery failures (on 4 different occasions), he's now done a similar thing which has again gone to Conduct. Any one else been dismissed for this type of thing? He has been counselled on numerous occasions.

Without knowing the reasons as to why he is failing , it's impossible for anyone to say.

Unless there is a really good reason for the latest failure, he will be lucky to defend himself from dismissal.
