Grievance Form.???

Can anyone give me some advice?
I want to put in a grievance against a colleague for B&H in the workplace, where do I get the H1 form from and who do I submit it too. I am not taking this measure lightly but after talking about this to my DOM on different occasions nothing changes and my victimization still continues.
