Sick pay clarification

Could someone give me 100% clarification on sick pay.
My question is when long term sick are you on full pay for the first six months of your absence irrelevant of previous absences or is it 6 months over a four year period ?
I've heard conflicting suggestions
Please can someone give me a up to date clarification.

I think its 6 months over any 4 year period starting from the first absence

6 months full pay and 6 months half pay over a 4 year period,cumulative.

What about 12 months full pay in cases of an accident on Duty??
6 months full then 6 months half pay is not the only option

Not 100% sure but I think if you go 12 months without going sick you start from scratch again so to speak.
