Depends entirely whether the customer complains it wasn't delivered, in which case it would be considered lost.

you say not recorded on the pda ? did you use a pda ? if so did the customer sign it and the number come up on the top of the screen ? it could be a missing text message

Could be you pressed the wrong option or the pda crashed. As mentioned all will depend if the customer claims they did not receive it.

Things like this happen and people seem to get away with it ok.

This will be classed as a technical failure, the PDA if used correctly will transmit in real time, has there been a Customer complaint about not receiving the Special??
