Accident Report falsified

I have just got a copy of the accident report complete by my Dom for an accident I had in September leading to hospitialisation and 3 months off sick and it
does not mention any of this. The date is incorrect, the location of the accident and my injury are not correct either.

I mentioned this to the Union H&S rep, and also my union guy and neither have been very positive about what can be done or if this can/will be changed.


I cant understand how this can have been done and not picked up by anyone - it even states on the form that only first aid given and no time off!

If your unit reps are not helping, go straight to your area h&s rep plus area union rep.

is it the ERICA output that you have that you think is wrong?

Your accident information should be contained in the ERICA completed/answered by you after the accident. You should have received, or can request a copy of that ERICA report for your own records. These are normally required by the business to have been completed within the first 24 hours, (unless the incident was followed by a non-service day).

You are technically not allowed to see the Managers Accident Investigation, but should be made aware of the resultant Root Cause Analysis upon its completion. This report is requested by the business to be completed within three days of the accident where possible, so you should have expected to have been given the 'deemed cause' shortly after.

That said, if you were hospitalised and absent for three months, then your accident was classed as a RIDDOR, it should have therefore been notified to the HSE and your ASR, who's also tasked with undertaking a full investigation. Do you not remember being interviewed by that ASR and performing an Accident Reconstruction with them too!

Your Union Area Safety Representative should have a complete record of the pertinent data, so contact them with your concerns in the first instance.

Still not seen an ERRICA report even though requested several times. Unfortunately Union Area Safety Rep is not very helpful, claiming
they have not seen any reports either.

DOM did visit me when I can out of hospital but I was still on morphine at the time for the pain and he was only interested in asking if
I was wearing the correct footwear (which I was).

Cant understand why no-one has picked this up and feel information about accident maybe being covered up so as not to report it- Area Rep said there was nothing he
could do for me so I asked him to escalate it further but have still heard nothing.

My DOM currently off sick so I cant even ask him

Should I report it further up the RM chain and if so to who and how got an email address for someone at royalmail.com but the POSTMASTER keeps
rejecting it.

Also this may sound daft but there is a reason for asking, why exactly is it you want them to correct the information?

Consider Emailing djoyce@cwu.org (CWU National Health & Safety Officer) and voice your immediate concerns directly with him. Make sure that you mention all of the above thereby giving him a clear indication of what exactly are the problems that you are facing. If your CWU Area Health and Safety Rep isn't in your opinion helping then he or she isn't performing their functions correctly.

They MUST report to the HSE within 15 days, if the accident results in an employee being unable to perform their normal work duties, for more than seven consecutive days as the result of their injury. (The seven day period does not include the day of the accident, but does include weekends and rest days).

It is also a requirement that accidents must be recorded if they result in a worker being incapacitated for more than three consecutive days.

In either case your Area Safety Rep needs to get his lazy arse into gear, or step down.

So, you were in a serious accident at work that led you to being in the hospital? And on painkillers even after you were released?

It sounds like you're not getting much helpful advice from your union. If you haven't already, I'd look at what other people from outside the business might have to say.

of me were injured are all wrong

By stating that I spent no time in hospital or even attended it has meant that the fact that they did not report the accident to the HSE as
a RIDDOR which I understand is an offence

The accident was cause my a paving slab breaking underfoot on delivery and I am trying to claim for out of pocket expenses and certain
loss of earnings all of which I cannot do properly without the correct documents and information according to my solicitor.

It seems wrong that someone somewhere will get away with this to save stats or are they hiding something more

seen a rta report but injury was due to paving slab breaking underfoot that doesnt make sense?

are you claiming against royal mail?

Initially I thought I had just hurt my ankle and shoulder in the fall (perhaps it was shock) but after
I hobbled back to the office (when I called in there was no vehicle to come and assist me) after being
told to finish my delivery I started to loose the feeling in my arm and could not hold anything between my
finger and thumb by the time I got home.

My wife took me to A&E as I have a prolapsed disk in my upper neck (another work injury sustained
several years ago), by this time I had total loss of feeling down my left arm and my ankle was badly swollen,
MRI and CAT Scan found no damage to existing disc injury but acute internal muscle and spinal bruising
which took its time to go down (over 6 weeks) with NHS acupuncture to stimulate nerves and relieve the pain
further MRI found no permanent damager to lower lumber discs (thankfully).

Feeling still comes and goes in hand and arm but went back to work Mid December and currently
doing REHAB
