If you have a hospital appointment do I have to take a days holiday or change my day off

It would depend on what the appointment is for and how understanding your manager is.

Hi I had a day of Friday for a morning appointment at hospital I took the letter in to show my manager the week before and he ok'ed it I don't know if he booked it as holiday or special leave but he was ok about it, as long as you give them notice and don't drop it on them the day before you should be ok.

you should be able to get time off
