Any idea where I can get a claim form for an accident?

i assume you are a cwu member.
you need to ask your legal and medical rep for an LS3 form.
if it was [an accident at work] during the course of your employment, you should have logged it in your wokplace "accident" book.
then you should contac the dwp [asap] and inform them of your "industrial" accident.
keep copies of everything and keep the cwu informed of everything.
