My stage warning was issued

ask your DIM, he will have a copy of it in your records folder.

You would have been given a letter to inform you of the issued stage warning and the date of when it was issued.

Presumably he's lost it (otherwise he'd know when it was). But without the letter, how can someone find out?
There must be a way.
