It's not on the web site ...that is the old DDA(DL56 form) which is now defunct, although in Royal Mails case it doesn't matter as the feckers never answer it anyway.. 
the new one you have to ring them up and either request it in paper form(ask for the new Equality Act 2010 Discrimination and other Prohibited Conduct Questions and Answer Forms) or if your very nice(just like me 
)they will send you an email with the attachment to print out.

Thank you both - the up to date one is on the website now so I've got it. Think I'll give it a go if they dont change their mind and agree reasonable adjustments this week.
