sick leave and stages info

i know it`s on here somewhere but due to personal reasons my heads spinning and can`t finnd what i`m after .
i`m after info on amount of sick leave that triggers stages , an official doc would be great that i could down load ,

4 instances or 14 days in any 12 month period, don't forget they include Sundays and Bank Holidays unless you ring in to say you're fit for work and will be back on ????day. Don't have the official document to hand but I'm sure fishtank or Postman will have it up within the hour.
