uniform order form

Anyone got a copy/or can point me to a link for the uniform order form so I can fill it in and then phone up. MY DOM gave me one two weeks ago and I filled it in and handed it back to them, and now he reckons it has been lost, and its best if I do it myself. Anyway I forgot to ask him for a new form, and I am off for the next two days. So I thought I might order it tomorrow, so anyone that can help, would be appreciated. :Very Happy

No I was told I can't have one as my initial contract was only three month. Now that I have been offered a new three month contract, I am allowed to have a uniform, wow arn't I lucky !
