Messed up holidays

Sorry decided to delete all the main message as I got worried hubby's manager might read and identify him.

Will just ask if anyone knows if managers are required to keep copies of holiday request forms in case of any errors made and if my hubby is within his rights to demand to see a copy of his to prove that he did in fact request the correct week off and it's the managers that have made a mistake.

they should retain a copy of their part of the holiday slip

As far as i can remember the form he's always had is just one A4 sheet of paper with the dates down one side and a box to tick to select which weeks you want, no part to be signed or returned. He's just always had verbal confirmation that the dates picked have been agreed. Never had any problem before in the three years he's been there.

in our office a holiday request is an A4 sheet
if permission isa given then the lower half is cut off and signed by the DIM
this has the dates on it(either what you requested or amended with whats available)
if its set annual leave then we have 6 A4 sheets covering the whole summer/winter period with everyones allocation on the sign on desk
