If the S1 warning was issued in Summer 2008 then it would have cleared off your record in Summer 2009. HOWEVER, you had an absence of 6 days in Dec 2008 and an absence of 6 days in November 2009, these are less than 12 moths apart and may trigger a fresh S1 warning if you have got your dates wrong as 14 days (or 4 short absences) in 12 months is the S1 standard.

If the S1 warning was issued in Summer 2009 (which technically is last Summer) then the S1 warning will be valid until Summer 2010, S2 should not be triggered as you state your Nov absence is less than 10 days.

you will be clear now mate , the poster above has got it spot on, just dont go sick again before the 7th december to clear your last lot of 6 days off your record and then you will only have 6days absence in the last 12 months which will be the most recent 1. That will give u 7 days further absence you can take in no more than 2 seperate absences for the rest of the 12 months before you trigger a stage 1 again.

Can you receive a stage one warning if you have been off with a accident at work

you shouldnt really, but it depends on the situation, a decent manager wouldnt issue you a stage but yes they can, but if ultimately it contributed to your dismissal because it triggered of future absences triggered a stage 3 , they would take that into account because you would always win an appeal if dismissed, 3 people this year won appeals for dismissals when they had accidents at work at my office.
