Has anyone used this form I seen on psp for subject access request?
Just that you cannot enter info using the drop down section in the yellow section.

As far as I am aware this option is not valid on PSP.
If you require information on yourself through a Data Subject Access Request (DSAR)
you telephone the Information Rights Team at HRSC in Sheffield as the information requested is personal and specific to yourself,
They will then email you the two forms required with instruction how to access your personal data.
