Symptoms after 10 days of self isolation

Apologies if this has been answered somewhere before...

I tested positive for Covid and self isolated for 10 days. I was due to go back to work yesterday, but I have diarrhoea which began on Saturday. Reading the NHS site, I should continue to self isolate and only stop after 48 hours of my symptoms ending.

Question is, do I need a self isolation note or a fit note?

If still covid related you will need a fit note.
