Anyone know how to fill this in ? apply for a postman, driving role, not sure how to fill it in.

You don't fill it in, it's already filled in.
Look at the role you're applying for, look at the boxes that are ticked and it will tell you what common hazards the role faces.
Look at Postman/woman and you can see that walking up to 8 miles a day is ticked, as is lone working, lifting and handling, standing/sitting etc. but working at heights of 2m or more, confined spaces etc aren't.
It's a reference document for yourself.
