E.R.I.C.A. cannot get a copy..........

......... The managers say they hold no record of the accident, yet all proper forms filled at the time etc, and said the h&s bloke has it but he keeps saying he'll copy it but never does (+ 6 months of asking).
Q: Would the D.W.P have a copy.
How can they not have records.

The injured party should be provided with a copy of the ERICA report upon completion however all ERICA reports can be printed out directly from the system at any time. All ERICA reports are accompanied by a Managers Accident Investigation report which are also available on the system, but not for viewing by the injured party, (The IP should however be informed of this investigations resultant Root Cause(s)).

They do have records and if your manager is doing his or her job responsibly, securely and correctly then a copy of your ERICA report should have been filed away in such a folder as this. If your manager has informed you that the health and safety bloke has it (CWU Area H&S Reps are meant to receive copies of all ERICA reports within their respective areas of responsibility), then he or she too will have a printed off copy filed away in said folder. I would certainly agree with Himaggen in that you should have received upon completion a copy of the report as well.

Suggest you liaise with your CWU workplace/unit h&s Rep to get the ball rolling or alternatively email Dave Joyce the CWU National Health, Safety and Environment officer djoyce@cwu.org and pose your problem to him directly.
