Testing prep and a testing dom

Your post suggests that the planners/DOM have put the revision in and then failed to complete phase 5 and 6,which would require the unit to change the signing on sheets to those within the phase 5 documents. These sheets have been designed to capture the difference from planned to actual, for example planned prep to actual prep and this info should then be transferred onto the main data sheet to identify the issue with the whole revision. This data would be used within the PIR process at weeks 4,8,12 and form the basis for any fixes that maybe required.
Although the IWT has improved, it's function isn't to measure individual performance and to do so would be outside of any agreements with the CWU.
The I/R process has time scales contained within it and think once you have put in the stage 1 the stage 2 should be held within 7 days, if the area rep has failed to deal with it because the DSM isn't playing ball then they should push this up to the Div reps. So guess you need to ask the area rep what they are doing with your disagreement and if not happy with the reply tell them that you will contact the outdoor dept concerning the lack of action on this disagreement.
Also inform the DOM that you will be contacting the ASR over his instruction to abandon the use of a trolley.
Some common reasons why the revision fail are: Planners didn't select LWT when building the walks, IWT performance set to high, fail to resource to the increased workload above the agreed model week, call rates to low, attendance call increases, poor frame layouts, failure to plan the safety outputs from wrap into the delivery design, and the list goes on and on.
Maybe ask the DOM for a copy of the good prep guide so you can check if the frames meet the standard.

That test is called a P40 test. All DOMs should do three per week without fail. The chance of a DOM actually doing any in a year is remote however for a revision to be happen, all delivery posties should have been put though a P40 test first.
