Re: INDUSTRIAL INJURY
There is a form you can fill in and post off. I did it just before Xmas after my AOD. You report it as if you're making a claim for benefit but you just don't fill in the bit to claim for benefits. Then, hopefully not, if anything does go wrong in years to come and it's related to that injury, at least you've reported it.
I think this is the page

Link for the form:

1, Inform your Supervisor/Manager? of your A.O.Dthe one wrote:WHEN YOU REPORT AN ACCIDENT ON DUTY,

2, Get A.O.D registered upon the business Erica System
3, During the Erica registration your Supervisor/Manager? will/should offer you a visit with the Employee Health Service (EHS) ATOS ORIGIN, which I advise you take, but what I don't advise is have a telephone consulation
4, Sign your copy of the Erica report and keep in a safe place for further refrence (when needed)
5, Contact your CWU Branch Secretary and request a Green LS3 form
6, Contact your Local Jobcentre Plus who will/should put you in contact with the DWP who will/should send you a form to register your A.O.D as an Industrial Injury and Industrial Injuries Disablement Benefit (if required)
7, The DWP will/should make there assessment regarding your A.O.D and if in your favor then "you" and only "you" will recieve from the DWP there decision.
You will then have to give a copy of the assessment to your Supervisor/Manager? and to the Business Accident Management Unit (AMU) Wolverhampton and to the CWU Branch Secretary.
I hope most or all of this helps

Re: INDUSTRIAL INJURY
what happens if you have to have an operation for restructuring of knee which has been ongoing but was never entered as an accident on duty?allthough the manner of the job meant it contributed greatly to the damage ?

Re: INDUSTRIAL INJURY
I believe then, your case should be delt with finally when all else fails by a Divisonal Rep (if dismissal is looming).darth postie wrote:what happens if you have to have an operation for restructuring of knee which has been ongoing but was never entered as an accident on duty?allthough the manner of the job meant it contributed greatly to the damage ?
On the bright side, your General Practictioner (GP) should have all records of your contition and how over time your job has contributed towards the damage.
Regular visits to the Employment Health Service (EHS) means the business have records of your degenerative knee condition and your Unit Manager(DOM) should also be aware of your condition, so they can make previsions for light duties upon your return for work.
Stage warnings will more than lightly be around the corner, but I'm positive your unit rep will be able to get this resinded, because who would want all the aggrovation of Grievance Procedures and the like.
Hope the operation goes well and be positive
