Advice re working hours please!

Does anyone have any experience of submitting a flexible working request? Do RM have a template, or should I just use the template from gov.co.uk?

I'm employed on the usual 3 month contract, 25 hours per week. Up until now Wed-Sat, 6.25 hours per day. I have been off on Maternity Leave since July last year, returned on Saturday to be told I am now working Mon-Sat, 5 hours per day, with a rolling day off. Actually I wasn't told, I checked the duty roster and noticed I had been allocated a round on Monday. A day I have never previously worked!!

I have been generously told I can work my original hours this week and next week.

Have they changed you job whilst you've been on maternity leave?
I thought that you had the right to return to your pre-maternity duty on your return to work.
