Yes they are different (I've used them both) get one or the other from your manager/rep.
Get one from your manager. They s**t themselves when you ask for one. I have asked for two in the past on different occasions from two different managers. They s**t themselves and do a complete u turn. Never had to fill one out, just asking for a form does the trick.

Lad in our office is experiencing something similar, own walk, far too big, goes over his time, boss wont pay him o/t, had the walk timed by another lino and it was way over, still refused him o/t. Im not sure if the lino scrubbed his o.t without telling him tho'.

So his grievance is in, what kind of grievance does he put in if it's not addressed within the week to kick it upstairs so to speak?

if you cant complete cut off everyday tell him no pay = no work and you won't complete until you get what you are owed.

exactly that, ive had many issues with them not paying me, and this happened around xmas last year, i didnt get payed for overtime so i stopped doing the work, at the time i was part time on a full time round, and we had alot of snow, i was leaving around 2 rows of the frame a day.. after the 3rd day they asked me why im doing this, once they settled what i was owed i continued to finish the duty, sometimes you have to cut your nose off to spite your face, it seemed to do the trick!
