Have you had blame-worthies before.
Has he taken you to task for H&S breaches.
Has he counselled you for your "blasé " attitude to H&S
Was this accident counted as a blameworthy

I hope, for you, that he has put this on to the stage 1 documentation and has not done the above as he is leaving himself open to criticism at any ET.

I also think in this case a Redress of Grievance would be a good move, as if he has not tackled your "attitude" before how can he with all conscience suddenly say that this was the cause of your sickness.

All accidents you have in a RM are your fault. Even if you weren't in the vehicle and it was hit by a drunk driver going the wrong way up a one way street in a tank... :crazy:
